Thank you for considering booking an appointment with me. Before you accept, please read through the the following.

1. I will not be able to prescribe your hormones. However, I will be able to make recommendations that can be brought to your doctor or nurse practitioner.
2. I will only be available for medical questions during your meeting with me and not via email or text in between meetings.
3. Cancellations made later than 48 hours prior to your meeting will be charged in full.
